The NP Dodge Company (the “Company”) prepares and maintains an IRS Form 1095-C for each full-time employee of the Company. The Form 1095-C provides information to you about the health insurance coverage offered to you by the Company.

Upon receipt of your request, the Company will provide your Form 1095-C the later of January 31 of the year following the calendar year for which the Form 1095-C was prepared, or within 30 days of receipt of your request.
